Urban Health Plan

Urban Health Plan (UHP) is a non-profit, federally qualified community health center system founded in 1974 by Dr. Richard Izquierdo to address the lack of accessible and affordable healthcare in the South Bronx. It has since grown into a large network with multiple locations in the Bronx, Corona, Queens, and Central Harlem, serving over 89,000 patients annually. UHP's mission is to continuously improve the health and quality of life of the communities it serves by providing comprehensive, high-quality, and affordable primary and specialty care. The organization is a 501(c)(3) nonprofit, licensed by the New York State Department of Health and accredited by the Joint Commission. El Nuevo San Juan Health Center, located at 1065 Southern Blvd, is the flagship location for Urban Health Plan. The name "El Nuevo San Juan" distinguishes it from the original "San Juan Health Center." This center is considered the heart of UHP's clinical operations, offering a wide array of services including adult medicine, pediatric care, women's health (OB/GYN), dental care, and specialty services such as cardiology, endocrinology, and podiatry. The clinic also provides essential support services like social services, health education, and assistance with health insurance enrollment. A key feature of El Nuevo San Juan Health Center is its commitment to culturally competent care, with a bilingual staff (primarily Spanish-speaking) and a focus on addressing the unique needs of the diverse communities it serves. The center expanded in 2014 with the addition of the Simpson Pavilion, which significantly increased its capacity for medical and diagnostic services.
