Oak Orchard Health is a well-established non-profit Federally Qualified Health Center (FQHC) that has been serving communities in Western New York since its founding in 1973. Originally established as a migrant health project, its roots run deep in the agricultural community, and it continues to provide specialized services for agricultural workers who often face barriers to quality healthcare, such as transportation, language barriers, and fear of losing work. With multiple locations spanning Monroe, Orleans, Wyoming, Steuben, and Genesee counties, including a health center in Hornell, NY, Oak Orchard Health offers a wide range of integrated healthcare services. These include comprehensive Primary Care for all ages, pediatric care, dental services, vision care, and Behavioral Health services (including mental health and addiction medicine). They also provide essential support services such as patient engagement, case management, health education, Spanish-English interpretation, and transportation assistance, all designed to ensure patients receive holistic and coordinated care. Oak Orchard Health is recognized as a Patient-Centered Medical Home (PCMH) by the National Committee for Quality Assurance (NCQA), signifying its commitment to high-quality, coordinated, and patient-centered care. In 2022, they also earned the Gold Seal of Approval from The Joint Commission, further reflecting their dedication to safe and quality patient care.
