The Harbor Care health and wellness Center is a non-profit, integrated healthcare and social services provider based in Nashua, New Hampshire. It operates as a Federally Qualified Health Center (FQHC), meaning it provides comprehensive primary care and other services to underserved populations, regardless of their ability to pay. The organization's roots trace back to 1980 under its former name, Harbor Homes. Harbor Care’s model is built on the philosophy of integrated care, recognizing that housing, healthcare, and other social services are interconnected. The center offers a wide range of services, including primary care, dental care, mental health counseling, substance use disorder treatment (including medication-assisted treatment), and a pharmacy.
