Originally established in 1999 as part of Washington County Hospital, the organization gained its Federally Qualified Health Center status in 2003 under its former name, Walnut Street community health center. In 2015, it relocated to its current, expanded facility at 201 S Cleveland Ave in Hagerstown, rebranding as Family Healthcare of Hagerstown to better reflect its comprehensive scope of services. FHH operates on a "patient-centered medical home" model, aiming to provide a wide array of integrated services under one roof. These services include: primary care: Staffed by board-certified physicians, nurse practitioners, and physician assistants, offering family medicine and pediatrics. dental care: Comprehensive dental services for children and adults, including preventive, restorative, and emergency care. They also run a "Healthy Smiles in Motion" mobile dental program that visits local schools and neighborhoods. behavioral health: Providing mental health services for both adults and children, with a team of psychiatrists, nurse practitioners, and therapists offering diagnostic assessments, medication management, individual and group therapy, and various therapeutic approaches like CBT and DBT.
