Family Health Services

Family Health Services is a non-profit, 501(c)(3) organization established in 2008, operating as a Federally Qualified Health Center (FQHC) in Sandusky, Ohio. Their mission is to provide comprehensive and accessible healthcare services, including Primary care, Dental services, and Behavioral health, to individuals and families regardless of their ability to pay. They serve residents across Erie, Ottawa, and Huron counties, utilizing a Sliding fee scale based on federal poverty guidelines to ensure affordability for uninsured, under-insured, and low-income patients. Beyond direct patient care, Family Health Services is committed to addressing the broader healthcare needs of the community. They strive to be a "healthcare home" for their patients, focusing on their physical, mental, spiritual, and social well-being. A significant part of their mission involves the education and training of future medical professionals, including medical students, residents, nurse practitioners, and medical assistants, contributing to the development of a skilled healthcare workforce. Family Health Services is dedicated to improving Community health outcomes, particularly in areas that have historically been underserved. They accept Medicare, Medicaid, and most commercial insurances, further enhancing accessibility. They operate multiple locations, including their E Water St. Senior Center location, a Hayes Ave location, and a Norwalk location, with a new Medical Center & Pharmacy at 149 E.
