Community Health Connections

Community Health Connections (CHC) is a non-profit, Federally Qualified community health center (FQCHC) serving the communities of North Central Massachusetts. Established in 2002, CHC's mission is to provide high-quality, affordable healthcare to the community, focusing on eliminating barriers to care. The organization operates multiple health centers, including the Fitchburg Community Health Center, and is committed to serving everyone, regardless of their income, insurance status, or ability to pay. The Fitchburg Community Health Center, located at 326 Nichols Road, is one of several locations offering a comprehensive suite of services. These services include primary medical care, preventive and restorative dental care, behavioral health services, substance use disorder treatment, optometry, and an optical shop. The clinic also has a lab and patient services department. CHC is recognized as a Patient-Centered Medical Home by the National Committee for Quality Assurance (NCQA), a model of care that prioritizes patient-centered relationships with clinical care teams. Community Health Connections is deeply involved in the local community through various partnerships. It collaborates with Fitchburg State University to provide healthcare services to all students, and with Mount Wachusett Community College to educate dental assistants and hygienists. The clinic also works with local organizations like Growing Places to host a mobile farmers market, and the Fitchburg Art Museum to showcase local art. CHC also participates in a Medicaid and Medicare Accountable Care Organization (ACO), Community Care Cooperative (C3), which helps to inform its strategies for delivering equitable, high-quality care. In addition to its land-based health centers, Community Health Connections has a program called ACTION Health Services that utilizes a land-based and mobile clinic strategy to provide services to the most underserved people in the area, including the homeless. The organization serves tens of thousands of patients annually across its various sites and is committed to providing compassionate and respectful care.
