Community Health Center of Cape Cod

Community Health Center of Cape Cod (CHC) is a comprehensive healthcare provider with a mission to improve community health by offering high-quality and compassionate care to everyone, regardless of their ability to pay. Originally founded in 1998 as the Falmouth Free Clinic to serve uninsured residents, it evolved into a Federally Qualified Health Center (FQHC) in 2003, expanding its services to include a broader patient base, including those with MassHealth, Medicare, and other insurance plans. The center operates on a "complete medical home" model, providing multidisciplinary and integrated health services under one roof. This approach helps patients overcome common barriers to care, such as financial, linguistic, cultural, and transportation issues. CHC offers a wide array of services, including primary care for all ages, women's health, dental care, optometry, behavioral health, substance use disorder services, and on-site affordable pharmacies. The organization also provides specialized programs, such as care coordination, a family resource center, and a veterans' partnership, to address specific community health challenges. With multiple locations throughout the Cape, including Falmouth, Mashpee, Bourne, Sandwich, and Centerville, CHC is committed to serving a broad regional area. As a non-profit, the center is supported by a combination of fees, grants, and philanthropic donations, and it offers a sliding fee scale based on income and family size for those who need it.
